This book presents the deterministic view of quantum mechanics developed by Nobel Laureate Gerard 't Hooft. Dissatisfied with the uncomfortable gaps in the way conventional quantum mechanics meshes with the classical world, 't Hooft has revived the old hidden variable ideas, but now in a much more systematic way than usual. In this, quantum mechanics is viewed as a tool rather than a theory. The book presents examples of models that are classical in essence, but can be analysed (...) by the use of quantum techniques, and argues that even the Standard Model, together with gravitational interactions, might be viewed as a quantum mechanical approach to analysing a system that could be classical at its core. He shows how this approach, even though it is based on hidden variables, can be plausibly reconciled with Bell's theorem, and how the usual objections voiced against the idea of 'superdeterminism' can be overcome, at least in principle. This framework elegantly explains - and automatically cures - the problems of the wave function collapse and the measurement problem. Even the existence of an "arrow of time" can perhaps be explained in a more elegant way than usual. As well as reviewing the author's earlier work in the field, the book also contains many new observations and calculations. It provides stimulating reading for all physicists working on the foundations of quantum theory. (shrink)

Superstring theory is an extension of conventional quantum field theory that allows for stringlike and branelike material objects besides pointlike particles. The basic foundations on which the theory is built are amazingly shaky, and, equally amazingly, it seems to be this lack of solid foundations to which the theory owes its strength. We emphasize that such a situation is legitimate only in the development phases of a new doctrine. Eventually, a more solidly founded structure must be sought.Although it is advertised (...) as a “candidate theory of quantum gravity”, we claim that string theory may not be exactly that. Rather, just like quantum field theory itself, it is a general mathematical framework for a class of theories. Its major flaw could be that it still embraces a Copenhagen view on the relation between quantum mechanics and reality, while any “theory of everything”, that is, a theory for the entire cosmos, should do better than that. Methods developed in a previous paper are employed to define an exact correspondence between the states of a deterministic cellular automaton in 1+1 dimensions and those of a bosonic quantum field theory. The result may be used to argue that quantum field theories may be much closer related to deterministic automata than what is usually thought possible.

Conventional particle theories such as the Standard Model have a number of freely adjustable coupling constants and mass parameters, depending on the symmetry algebra of the local gauge group and the representations chosen for the spinor and scalar fields. There seems to be no physical principle to determine these parameters as long as they stay within certain domains dictated by the renormalization group. Here however, reasons are given to demand that, when gravity is coupled to the system, local conformal invariance (...) should be a spontaneously broken exact symmetry. The argument has to do with the requirement that black holes obey a complementarity principle relating ingoing observers to outside observers, or equivalently, initial states to final states. This condition fixes all parameters, including masses and the cosmological constant. We suspect that only examples can be found where these are all of order one in Planck units, but the values depend on the algebra chosen. This paper combines findings reported in two previous preprints (G. ’t Hooft in arXiv:1009.0669 [gr-qc], 2010; arXiv:1011.0061 [gr-qc], 2010) and puts these in a clearer perspective by shifting the emphasis towards the implications for particle models. Most attempts at obtaining theories that unify quantum mechanics with general relativity require violation of locality and/or causality to some degree. Here, we suggest that these problems actually are very deep and fundamental; hence they may require reconsideration of both quantum mechanics and general relativity at a very fundamental level. An approach called ‘deterministic quantisation’ is sketched.

Matter interacting classically with gravity in 3+1 dimensions usually gives rise to a continuum of degrees of freedom, so that, in any attempt to quantize the theory, ultraviolet divergences are nearly inevitable. Here, we investigate matter of a form that only displays a finite number of degrees of freedom in compact sections of space-time. In finite domains, one has only exact, analytic solutions. This is achieved by limiting ourselves to straight pieces of string, surrounded by locally flat sections of space-time. (...) Globally, however, the model is not finite, because solutions tend to generate infinite fractals. The model is not (yet) quantized, but could serve as an interesting setting for analytical approaches to classical general relativity, as well as a possible stepping stone for quantum models. Details of its properties are explained, but some problems remain unsolved, such as a complete description of the most violent interactions, which can become quite complex. (shrink)

Fast moving classical variables can generate quantum mechanical behavior. We demonstrate how this can happen in a model. The key point is that in classically evolving systems one can still define a conserved quantum energy. For the fast variables, the energy levels are far separated, such that one may assume these variables to stay in their ground state. This forces them to be entangled, so that, consequently, the slow variables are entangled as well. The fast variables could be the vacuum (...) fluctuations caused by unknown super heavy particles. The emerging quantum effects in the light particles are expressed by a Hamiltonian that can have almost any form. The entire system is ontological, and yet allows one to generate interference effects in computer models. This seemed to lead to an inexplicable paradox, which is now resolved: exactly what happens in our models if we run a quantum interference experiment in a classical computer is explained. The restriction that very fast variables stay predominantly in their ground state appears to be due to smearing of the physical states in the time direction, preventing their direct detection. Discussions are added of the emergence of quantum mechanics, and the ontology of an EPR/Bell Gedanken experiment. (shrink)

Standard canonical quantum mechanics makes much use of operators whose spectra cover the set of real numbers, such as the coordinates of space, or the values of the momenta. Discrete quantum mechanics uses only strictly discrete operators. We show how one can transform systems with pairs of integer-valued, commuting operators $P_i$ and $Q_i$ , to systems with real-valued canonical coordinates $q_i$ and their associated momentum operators $p_i$ . The discrete system could be entirely deterministic while the corresponding (p, q) system (...) could still be typically quantum mechanical. (shrink)

Studies in the neurobiological underpinnings of social information processing bypsychologists, neurobiologists, psychiatrists, radiologists, and neurologists, using methods thatrange from brain imaging techniques to comparative analyses.

Hawking particles emitted by a black hole are usually found to have thermal spectra, if not exactly, then by a very good approximation. Here, we argue differently. It was discovered that spherical partial waves of in-going and out-going matter can be described by unitary evolution operators independently, which allows for studies of space-time properties that were not possible before. Unitarity dictates space-time, as seen by a distant observer, to be topologically non-trivial. Consequently, Hawking particles are only locally thermal, but globally (...) not: we explain why Hawking particles emerging from one hemisphere of a black hole must be 100 % entangled with the Hawking particles emerging from the other hemisphere. This produces exclusively pure quantum states evolving in a unitary manner, and removes the interior region for the outside observer, while it still completely agrees locally with the laws of general relativity. Unitarity is a starting point; no other assumptions are made. Region I and the diametrically opposite region II of the Penrose diagram represent antipodal points in a PT or CPT relation, as was suggested before. On the horizon itself, antipodal points are identified. A candidate instanton is proposed to describe the formation and evaporation of virtual black holes of the type described here. (shrink)

In the standard formalism of quantum gravity, black holes appear to form statistical distributions of quantum states. Now, however, we can present a theory that yields pure quantum states. It shows how particles entering a black hole can generate firewalls, which however can be removed, replacing them by the ‘footprints’ they produce in the out-going particles. This procedure can preserve the quantum information stored inside and around the black hole. We then focus on a subtle but unavoidable modification of the (...) topology of the Schwarzschild metric: antipodal identification of points on the horizon. If it is true that vacuum fluctuations include virtual black holes, then the structure of space-time is radically different from what is usually thought. (shrink)

A promising strategy for better understanding space and time at the Planck scale, is outlined and further pursued. It is explained in detail, how black hole unitarity demands the existence of transformations that can remove firewalls. This must then be combined with a continuity condition on the horizon, with antipodal identification as an inevitable consequence. The antipodal identification comes with a \ inversion. We claim to have arrived at ‘new physics’, but rather than string theory, our ‘new physics’ concerns new (...) constraints on the topology and the boundary conditions of general coordinate transformations. The resulting theory is conceptually quite non trivial, and more analysis is needed. A strong entanglement between Hawking particles at opposite sides of the black hole is suspected, but questions remain. A few misconceptions concerning black holes, originating from older investigations, are discussed. (shrink)

It is put forward that modern elementary particle physics cannot be completely unified with the laws of gravity and general relativity without addressing the question of the ontological interpretation of quantum mechanics itself. The position of superstring theory in this general question is emphasized: superstrings may well form exactly the right mathematical system that can explain how quantum mechanics can be linked to a deterministic picture of our world. Deterministic interpretations of quantum mechanics are usually categorically rejected, because of Bell’s (...) powerful observations, and indeed these apply here also, but we do emphasize that the models we arrive at are super-deterministic, which is exactly the case where Bell expressed his doubts. Strong correlations at space-like separations could explain the apparent contradictions. (shrink)

BackgroundConsent for data research in acute and critical care is complex as patients become at least temporarily incapacitated or die. Existing guidelines and regulations in the European Union are of limited help and there is a lack of literature about the use of data from this vulnerable group. To aid the creation of a patient-centred framework for responsible data research in the acute setting, we explored views of patients and next-of-kin about the collection, storage, sharing and use of genetic and (...) health-related data for observational research.MethodsWe conducted qualitative interviews (n = 19) with Dutch sudden cardiac arrest survivors who donated clinical and socio-economic data and genetic samples to research. We also interviewed their next-of-kin. Topics were informed by ethics literature and we used scenario-sketches to aid discussion of complex issues.ResultsSudden cardiac arrest survivors displayed limited awareness of their involvement in health data research and of the content of their given consent. We found that preferences regarding disclosure of clinically actionable genetic findings could change over time. When data collection and use were limited to the medical realm, patients trusted researchers to handle data responsibly without concern for privacy or other risks. There was no consensus as to whether deferred consent should be explicitly asked from survivors. If consent is asked, this would ideally be done a few months after the event when cognitive capacities have been regained. Views were divided about the need to obtain proxy consent for research with deceased patients’ data. However, there was general support for the disclosure of potentially relevant post-mortem genetic findings to relatives.ConclusionsSudden cardiac arrest patients’ donation of data for research was grounded in trust in medicine overall, blurring the boundary between research and care. Our findings also highlight questions about the acceptability of a one-time consent and about responsibilities of patients, researchers and ethics committees. Finally, further normative investigation is needed regarding the (continued) use of participants’ data after death, which is of particular importance in this setting. Our findings are thought to be of relevance for other acute and life-threatening illnesses as well. (shrink)

A diagnosis of dementia often comes with difficulties in understanding a conversational context and expressing how one feels. So far, research on how to facilitate advance care planning for people with dementia focused on defining relevant themes and topics for conversations, or on how to formalize decisions made by surrogate decision makers, e.g., family members. The aim of this review is to provide a better scope of the existing research on practical communication aspects related to dementia in ACP conversations. In (...) November 2020, seven databases were searched to select papers for inclusion. This search was updated in December 2021. The search strategy consisted of three tiers, intersected by using the Boolean term “AND,” and resulted in 787 studies. Two researchers followed explicit criteria for two sequential levels of screening, based on titles and abstracts and full papers. A total of 22 studies were included for data analysis. Seven topics emerged clustered around two themes. This scoping review provides practical suggestions for healthcare professionals to improve ACP communication and uncovered gaps in research on communication aspects related to dementia in ACP conversations, such as non-verbal behavior, timing and implementation, and personal preferences. Background: More and more quantitative information is becoming available about the risks of complications arising from medical treatment. In everyday practice, this raises the question whether each and every risk, however low, should be disclosed to patients. What could be good reasons for doing or not doing so? This will increasingly become a dilemma for practitioners.Objective: To report doctors’ views on whether to disclose or withhold information on low risks of complications.Methods: In a qualitative study design, 37 respondents were included. (...) Focus group interviews were held with 22 respondents and individual in-depth interviews with 15.Results: Doctors have doubts about disclosing or withholding information on complication risk, especially in a risk range of 1 in 200 to 1 in 10 000. Their considerations on whether to disclose or to withhold information depend on a complicated mix of patient and doctor-associated reasons; on medical and personal considerations; and on the kind and purpose of intervention.Discussion: Even though the degree of a risk is important in a doctor’s considerations, the severity of the possible complications and patients’ wishes and competencies have an important role as well. Respondents said that low risks should always be communicated when there are alternatives for the intervention or when the patient may prevent or mitigate the risk. When the appropriateness of disclosing risks is doubtful, doctors should always tell their patients that no intervention is without risk, give them the opportunity to gather all the information they need or want, and enable them to detect a complication at an early stage. Many individuals with developmental language disorder (DLD) and individuals who are deaf or hard of hearing (D/HH) have social–emotional problems, such as social difficulties, and show signs of aggression, depression, and anxiety. These problems can be partly associated with their executive functions (EFs) and theory of mind (ToM). The difficulties of both groups in EF and ToM may in turn be related to self-directed speech (i.e., overt or covert speech that is directed at the self). Self-directed speech is thought to (...) allow for the construction of non-sensory representations (i.e., representations that do not coincide with direct observation). Such non-sensory representations allow individuals to overcome the limits set upon them by the senses. This ability is constrained by the development of word meaning structure (i.e., the way words are understood). We argue that the greater ability to construct non-sensory representations may result in more enhanced forms of EF and ToM. We conclude that difficulties in EF, ToM, and social–emotional functioning in those with hearing and language problems may be accounted for in terms of word meaning impairments. We propose that word meaning structure and self-directed speech should be considered in assigning EF and ToM treatments to individuals with DLD and those who are D/HH. (shrink)

Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home-sent (...) questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

Our approach to emotion emphasized three key ingredients. We do not yet have a mature science of emotion, or even a consensus view—in this respect we are more hesitant than Sander, Grandjean, and Scherer or Luiz Pessoa. Relatedly, a science of emotion needs to be highly interdisciplinary, including ecology, psychology, neuroscience, and philosophy. We recommend a functionalist view that brackets conscious experiences and that essentially treats emotions as latent variables inferred from a number of measures. But our version of functionalism (...) is not definitional or ontological. It is resolutely methodological, in good part because it is too early to attempt definitions. (shrink)
